How we use your intake

In Context Pty Ltd (ABN 54 700 208 920) collects these details to review your request, check service suitability and contact the person you nominate. We record your answers, consent, the notice version and the submission date. This is an initial intake, not a clinical assessment or an agreement to treatment.

Your answers are stored in our private website database. Access is restricted to the authorised site owner. A notification is sent to info@incontext.au with a reference and a sign-in link; your answers are not included in that email. Our website uses OpenAI Sites and Cloudflare, and notifications use Resend. These providers may process information outside Australia.

This form does not subscribe you to marketing or give blanket permission to share your information with other providers. We will discuss any further information sharing with you, unless disclosure is required or permitted by law.

Optional questions can be left blank. We need the required contact details and consent to respond through this form. You can use a first name or pseudonym for an initial enquiry where practical; we will explain if further identification is needed before providing care.
